At Seraphic Wellness Center, we are committed to protecting your privacy, maintaining confidentiality, and ensuring clear communication regarding medication and records policies. Please review the following information carefully.

Prescription Refill Policy

  • Medication refills are typically addressed during your scheduled appointment. To ensure accurate and safe prescribing, please bring an updated list of your current medications to each visit.
  • Refill requests must be submitted at least 72 hours in advance. Long term or maintenance medications generally require a follow up appointment before additional refills are authorized.
  • For most refill requests, patients should first contact their pharmacy. The pharmacy will send a refill authorization request directly to our office with the necessary information. It is the patient’s responsibility to inform the provider during the appointment if medication supplies are running low to prevent interruption in treatment.
  • If you use a mail order pharmacy service, you are responsible for submitting any required forms after they are completed by our office to avoid delays.
  • Refills should be managed during regular appointments whenever possible. In unexpected situations where medication runs out, please contact the office during normal business hours for guidance.
  • Non controlled medications may be refilled if a follow up appointment is scheduled or arranged. Controlled substance prescriptions that are reported lost or stolen may require documentation such as a police report and will only be replaced if deemed clinically appropriate and safe.
  • Please note that authorization of a refill does not guarantee insurance coverage. Patients are responsible for confirming medication benefits and coverage directly with their pharmacy or insurance provider.

Medical Records and Confidentiality Policy

  • Seraphic Wellness Center prioritizes patient confidentiality and complies with all applicable privacy regulations, including HIPAA guidelines.
  • Medical records are securely maintained in accordance with federal and state privacy standards. The release of any medical information requires a completed and signed authorization form from the patient before records can be shared with any outside party.
  • Requests for copies of medical records for personal use, continuity of care, disability documentation, or other purposes may require administrative review and approval. A professional processing fee may apply for preparation and duplication of records as permitted by law.
  • Please be advised that clinical visits are intended for assessment, treatment planning, and symptom management. Appointments are not conducted for the purpose of legal determinations, disability evaluations, competency assessments, or court related documentation unless otherwise formally arranged in advance.
